What you have to do in order to get a photo ID card with address proof is, just obtain the form for address proof from your nearest post office by paying Rs. 10. After filling up the form submit it in the post office along with a fee of Rs. 240. Since this postal ID card is not much popular, it is possible that the application form will not be available at your nearest post office. They will say you 'we will bring it for you from Head Office'. The unavailability of forms and lack of popularity of this card may cause it to take time to get your Address-ID proof. But if you don't want to wait, You may Download the Application form here.

A postman is sent to your address by the Postal Department to verify the address you provided. After the verification of the address provided through application form is over you will get your address id card. Note that the entire process can take up to 2 months, though it also depends on the efficiency of your post office.
